HEALTH INSURANCE INFORMATION

Many health insurance companies cover nutrition services. Some policies will only cover nutrition counseling if you have been diagnosed with certain medical conditions, while other policies may cover nutrition counseling for prevention (aka preventative benefits). We recommend contacting your insurance company to verify the details about what’s covered on your specific plan.

How to verify your benefits:

• Call the 800 number on the back of your insurance card and ask them the questions on the Verification of Benefits form

• Be sure to get the representative’s name and a reference number

• Once you have verified your benefits, email the completed Verification of Benefits form to admin@radrdnutrition.com and we will be in touch about scheduling your Initial Consultation 

• If your insurance plan requires a physician referral for nutrition counseling services, please contact your doctor and have them fax the referral to 630-382-8600
